DCON Code of Conduct/Medical Form

The District Board requires all attendees (members, chaperons, and guests) to submit both the Code of Conduct and Medical Form. Every member must complete and submit their forms with physical signatures. For submitting forms please following theses steps

1. Download and share both forms with attendees.
2. Have attendees complete forms with physically signed signatures.
3. Scan the files into PDFs on your phone/computer.
4. Find/Create a folder for your club in the DCON ATTENDEES FORM Google Folder.
5. Find/Create Medical Form Folder and upload all attendee forms.
6. Find.Create Code of Conduct Form and upload all attendee forms.
7. DONE!!!
